Immunization Letters

Letters have been sent out to those 6th graders who - according to our records - do not have the required immunizations for 7th grade.  Although this is a very early notice (one year ahead!) it gives plenty of time to obtain the immunizations before the next school year.  Students who do not have the appropriate immunizations for 7th grade (Tdap and MCV) will not be able to start school in August 2018.  If you have received a letter, please send in your  immunization record as soon as the immunizations are received!

6th and 7th graders --- Exam Forms!

It's time to turn in 6th grade physical forms and 7th grade dental forms!
These exams are mandated by the State of Pennsylvania, and must be completed for school attendance.  Examinations from the past calendar year can be accepted.  Find the blank forms on our website.
Students may also see the school doctor or dentist at no charge.  Please contact the school nurse for information.

Flu Clinic at UHS

Be sure to attend this year's flu clinic at the Unionville High School cafeteria on Thursday, October 5th.  Open to the public from 3PM until 7 PM.  Watch for details on the UCFSD website.

Letter to 6th grade Parents

Keep an eye out for a letter coming home with your 6th grader!

This letter explains important health information for 7th grade : immunization requirements, PIAA information, and the dental examination requirement.  (Feel free to turn in the 7th grade dental exam at any time).
